Applicant understands participation in the Pivotal Stock Lens Program requires a free membership with Pivotal Group before the stock account can be activated. Standard terms of payment are due by the 15th of the following statement month. Automatic payments are required for an active account and are processed by credit card or ACH on file by the 10th of each month. A late charge of $25, or 5.0% of the current balance, whichever is larger, per month is assessed after the due date if payment is not able to be processed on time. Pivotal group reserves the right to amend the conditions of this agreement with a written notice. Return check charge is $25.00 plus any handling or legal fees.

I, the undersigned, hereby agree that the company/corporation on this application will acquire full responsibility that in the event of default of any amount due, and should this account be transferred to an agency, attorney fees and the court costs incurred and permitted by the laws governing these transactions.

I, the undersigned, for and in consideration of your extending credit at my request to the company named on this application (hereinafter referred to as the "Company"), of which I am of Title as below, hereby personally guarantee to you the payment to Pivotal Group in the State of Texas of any obligation of the Company and I hereby agree to bind myself to pay you on demand any sum which may become due to you by the Company whenever the company shall fail to pay the same. It is understood that this guaranty shall be a continuing and irrevocable guaranty and indemnity for such indebtedness of the Company.

I do hereby waive notice of default, non-payment and notice thereof and consent to any modification or renewal of the credit agreement hereby guaranteed. The undersigned declares that they are authorized to execute contracts for this business and have completed this application fully and truthfully. The undersigned also authorizes the holder of this application to investigate the firm's credit history. No application will be processed without signature.
